The lock should be aligned on a cache line boundary so it isn't in a
cache line with other data. This is important for 'tdq_lock' as it is
used by every processor which constantly acquires and releases it. This
alignment can reduce how violently cache lines bounce between processor
local caches.

This commit adds a new routine called tty_attach(). This routine allows
a TTY to be registered with the system and have a device file associated
with it.

Holding down the caps-lock key would result in the LED blinking rapidly
which is obviously not normal. This commit corrects that behavior.

Physical memory pages to be allocated is 'len / GRANULE'. Previously, if
'len' was less than the machine page-size, the amount of pages to allocate
would be 0 causing the map to fail.
